No one wants to get caught discriminating against the disabled. Use this self-evaluation form, courtesy of Candi Atkins of Candi Atkins Consulting in Las Vegas, to make sure you have your 504 bases covered.
OVERVIEW
1. List the personnel responsible for coordinating Section 504 Compliance: __________________________.
2. List the steps taken to ensure that all staff involved in any aspect of this property are informed periodically of, and understand fully, the policy of nondiscrimination on the basis of handicap.
Example: Initial training from the property manager; signed employee non-discrimination policies; periodic workshops; memorandums and articles that explain Fair Housing, American With Disabilities Act and Section 504 laws; availability of HUD Handbook 4350.3 for easy reference.
Instructions: For each question below, answer either YES or NO. If your response is NO, then identify how you will modify policies and/or practices or what corrective action you will take. In some cases, a YES answer will also require that you supply additional information to complete the evaluation process.
Tip: If any changes are needed, create a transition plan that outlines what is needed, when it will be completed (the target date), the cost and the funding source (e.g., operations, replacement reserve).
